When completing your intake forms, please note that the primary client should be the lactating person. Support people (partners, spouses, family members) are absolutely welcome to attend appointments, but they do not need to be added as separate clients in the system.

For charting and privacy purposes, only one email address should be linked to the primary client. If someone else is helping you complete the intake forms, please have them use the intake link sent directly to the primary client’s email.

This helps keep your chart accurate and ensures seamless care. Thank you for your cooperation—I can’t wait to work with you.
